What Can I Expect in the Initial Consultation?

Our office environment is professional, relaxed and welcoming. We are committed to collaborating with you to decrease symptoms that cause you distress – with treatment modalities that are safe, efficient and effective.

To begin treatment you will need to complete an information form. The form is available via download or can be completed upon arrival at our office. A thorough diagnostic interview, including conversation, will be used to establish an initial diagnosis. A second session may be necessary to complete the diagnostic history and determine a plan of treatment. Depending on the assessment, you may return for additional follow-up sessions that aid in your recovery.

Punctuality is essential. Plan to arrive 15 minutes before your scheduled consultation to complete all necessary paperwork.

Payment or co-payment or co-insurance is due at the time of your visit.
Payment may be made with cash or personal check(s).

Please know that healing and change require commitment and self-discipline.

Fees and Payment Information

Appointment Fees:

The fee for a 45-minute psychotherapy session is $125. We ask for payment at each session. If your insurance covers our sessions, you are responsible for any co-pay, co-insurance, and deductible charges. If you have a PPO for health insurance, we will provide you with the necessary paperwork, and you will be reimbursed directly by your insurance company. My license is Licensed Independent Clinical Social Worker (LICSW).

If your insurance company is not one of the above, please do not be deterred from contacting us, as many plans provide out-of-network benefits and we are committed to to making therapy affordable for you. A phone call to your health insurance company can obtain that information. We are also available to discuss self-pay at a fee commensurate with your financial ability.

Late Arrivals and Cancellation Fees:

If you arrive late for an appointment, you will be seen for the remaining time and charged for the full session as scheduled.

Cancellations must be made no later than 24 hours of the scheduled appointment. Otherwise, a late cancellation fee of $100.00 will be assessed.

Missed Appointments:

If you miss an appointment with no notice, a no show fee of $100 will be assessed.

Paperwork/Administrative Fees:

Disability or other paperwork not directly related to your therapy will not be completed during your session. If you need assistance with administrative forms or treatment confirmation documentation for school, employer, RMV, court, attorney or disability, you will be billed at hourly rate of $150, tracked in 15-minute increments, with a minimum charge of $50.

Good Faith Estimate:

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
